Lifestyle diseases constitute an increasing proportion of health problems and this trend is likely to continue. A better understanding of the responsibility argument is important for the assessment of policies aimed at meeting this challenge. Holding individuals accountable for their choices in the context of health care is, however, controversial. There are powerful arguments both for and against such policies. In this article the main arguments for and the traditional arguments against the use of individual responsibility as a criterion for (...) the distribution of scarce health resources will be briefly outlined. It is argued that one of the most prominent contemporary normative traditions, liberal egalitarianism, presents a way of holding individuals accountable for their choices that avoids most of the problems pointed out by the critics. The aim of the article is to propose a plausible interpretation of liberal egalitarianism with respect to responsibility and health care and assess it against reasonable counter-arguments. (shrink)

Genomics provides information on genetic susceptibility to diseases and new possibilities for interventions which can fundamentally alter the design of fair health policies. The aim of this paper is to explore implications of genomics from the perspective of equal opportunity ethics. The ideal of equal opportunity requires that individuals are held responsible for some, but not all, factors that affect their health. Informational problems, however, often make it difficult to implement the ideal of equal opportunity in the context of healthcare. (...) In this paper, examples are considered of how new genetic information may affect the way individual responsibility for choice is assigned. It is also argued that genomics may result in relocation of the responsibility cut by providing both new information and new technology. Finally, how genomics may affect healthcare policies and the market for health insurance is discussed. (shrink)

The origin and development of Punnett’s Square for the enumeration and display of genotypes arising in a cross in Mendelian genetics is described. Due to R. C. Punnett, the idea evolved through the work of the ‘Cambridge geneticists’, including Punnett’s colleagues William Bateson, E. R. Saunders and R. H. Lock, soon after the rediscovery of Mendel’s paper in 1900. These geneticists were thoroughly familiar with Mendel’s paper, which itself contained a similar square diagram. A previously-unpublished three-factor diagram by Sir Francis (...) Galton existing in the Bateson correspondence in Cambridge University Library is then described. Finally the connection between Punnett’s Square and Venn Diagrams is emphasized, and it is pointed out that Punnett, Lock and John Venn overlapped as Fellows of Gonville and Caius College, Cambridge. A special issue on ‘Private Autonomy and Public Paternalism’ constitutes the first part of this issue. Guest-editors are Annette Dufner and Michael Kühler, both from the University of Munster, Germany. It is often assumed that personal autonomy is a ‘private’ matter in the sense that it is based primarily on a person’s subjective characteristics and capabilities. At the same time, the literature mainly deals with paternalism as a problem of the ‘public’ sphere, for example by focusing on the dangers that (...) threaten the autonomy of individuals as citizens, such as state paternalism. However, it is widely acknowledged nowadays that personal autonomy can only develop and flourish if conditions in the social and relational sphere are favourable, which means that personal autonomy is not so private after all. At the same time, it should be clear that paternalism not only relates to our behaviour in the public sphere, but also to how we behave in more private social spheres, like family, frien. (shrink)

Hamiltonian light-front dynamics of quantum fields may provide a useful approach to systematic nonperturbative approximations to quantum field theories. We investigate inequivalent Hilbert-space representations of the light-front field algebra in which the stability group of the light front is implemented by unitary transformations. The Hilbert space representation of states is generated by the operator algebra from the vacuum state. There is a large class of vacuum states besides the Fock vacuum which meet all the invariance requirements. The light-front Hamiltonian must (...) annihilate the vacuum and have a positive spectrum. We exhibit relations of the Hamiltonian to the nontrivial vacuum structure. (shrink)

The assessment of decision-making capacity in patients with brain injuries presents a range of clinical and legal challenges. Existing guidance on the conduct of such assessments is often generic; guidance specific to patients with brain injury is sparse and coarse-grained. We report on an interview-based study of decision-making capacity in patients suffering from acquired brain injury and organic personality disorder. We identify challenges associated with the assessment of DMC in this patient population, review three bodies of relevant research from cognitive (...) neuropsychology and neurophysiology, and draw on phenomenological analysis to identify three distinct abilities that play a role in decision-making, but which can be compromised in patients with organic personality disorder. We address the challenge of translating clinical findings into legally attestable results. (shrink)

When a single beam-splitter receives two beams of bosons described by Fock states (Bose-Einstein condensates at very low temperatures), interesting generalizations of the two-photon Hong-Ou-Mandel effect take place for larger number of particles. The distributions of particles at two detectors behind the beam splitter can be understood as resulting from the combination of two effects, the spontaneous phase appearing during quantum measurement, and the quantum angle. The latter introduces quantum “population oscillations”, which can be seen as a generalized Hong-Ou-Mandel effect, (...) although they do not always correspond to even-odd oscillations. (shrink)
